Well, as it is now, I can't do that either. I haven't checked it closer, but there its something that makes Painkiller lag.
So I just turned almost every video option on low or off. So now its runs smooth.
Maybe I should check if there is something I can have turned on or at least at medium setting.

Certainly there are things like that. I have quite a few options set to medium or even high, and my computer is not a really strong machine. Mainly I have two profiles - for deathmatch I have most settings set to low, like you do, but for sp I change them to higher values.
What takes most of your CPU is dynamic lighting and shadows, you can safely play with other options and see how it works. Architecture for example, or textures, try setting them to medium or high. And play with 1024x768 resolution at least.
